Children's Long Term Support Program
We proudly partner with the State of Wisconsin to offer essential support and services to children with disabilities and their families. Our home and community based program is designed to empower children to grow and thrive, enabling them to live their best lives.Through personalized care and a supportive environment, we are committed to making a meaningful impact on the lives of the families we serve.
Services
We provide services such as teaching daily living skills, Empowerment and self determination support and mentoring
Eligibility
- Be eligible for Wisconsin Medicaid
- Be younger than 22 years old
- Live at home, in foster care, or in another approved setting
- Need a level of care that people get at: hospital, a nursing home, an institution for people with developmental disabilities.
- Be able to get safe, required care at home or in the community.
